Dr. Moore is on staff with both Baptist and Conway Regional.

Does Dr. Moore treat pediatrics?

Yes! The clinic treats may pediatric conditions, including warts, ingrown toenails, flat feet, sports injuries, and more!

What insurances are accepted at CAFAS?

Aetna, Anthem, BCBS (Commercial and Federal), Centene/Qualchoice, Cigna, Trustmark, Union Pacific, Zelis, United, Municipal Health, Ambetter/Allwell, Tricare, Medicare-Railroad, Arkansas Medicaid, VA Community Care, Humana, and Arkansas PASSE. If you don’t see your insurance listed here, give us a call to confirm your insurance acceptance!

Do I need a referral to make an appointment?

Most insurance do not require referrals! The most common exceptions to this are Medicaid and Tricare. If you are unsure if your insurance requires a referral, give us a call!

How much will a visit cost?

Depending on your insurance carrier, copay, co-insurance, deductible, and what services are rendered, the cost for your visit will vary. We can estimate costs for services, so feel free to ask our staff to do so prior to receiving treatment!
